Standards & Universal Preferences
Standing principles and universal preferences that apply across this program.

Preference & Supply Program · Pediatric Orthopaedic Trauma (non-spine) · Working reference
Purpose
These preference cards are written for the circulating nurse, scrub technician, PA, resident, and OR team. They are intentionally practical: what to open, what to have available, and what not to waste. Each procedure card opens with its CPT / booking codes, then walks through positioning, equipment, instruments, implants, and dressing. The companion Set Library spreadsheet lists tray contents; the companion Supply Catalog lists par-stock consumables, implants, bracing, and clinic supplies with vendor and catalog detail.
GLOBAL PRINCIPLES
- Keep pediatric cases lean unless case complexity demands escalation.
- Use the Pedipod War Chest (Card 5) as the universal orthopaedic add-on for pediatric fractures, open reductions, and open-fracture / I&D cases — except for percutaneous pinning (CRPP) cases.
- Smooth K-wires only for routine percutaneous pinning; no threaded K-wires unless specifically requested.
- Cap every exposed wire / pin end — rubber pin covers on all protruding K-wires and Steinmann pins.
- Trauma line is Globus — use Globus implants first where a system exists (ANTHEM plating, CAPTIVATE cannulated screws); fall back to peds-specific lines (OrthoPediatrics, DePuy) where Globus has no pediatric option (e.g., flexible nails).
- Skin closure: use SYLKE silk-fibroin closure in place of Steri-Strips when possible.
- When a specialized instrument is requested, do not substitute by name alone — match geometry, size, and intended use.
- Reference standard, not brand mandate: a facility may substitute an equivalent product from any vendor provided it matches the listed specification, size, and function.
CPT NOTE
CPT codes below are the reference / booking standard for the most common pediatric presentations and assume the typical technique noted. Confirm laterality (RT/LT) and the exact code against the operative findings, the current CPT book, and payer policy before billing. Codes are reported per case as appropriate; percutaneous fixation, open treatment, and debridement codes are not interchangeable.
Appendix — Companion References
- Set Library: Wey_Peds_Ortho_Set_Library.xlsx — full tray contents and counts for the Pedipod War Chest, pinning, soft-tissue, I&D, and cast-room setups, plus implant systems.
- Supply Catalog: Wey_Peds_Ortho_Supply_Catalog.xlsx — master par-stock list of consumables, implants, casting, bracing / DME, and patient-comfort items with vendor, catalog #, size, stock location, and status.
- Entries marked TBD require final confirmation by Materials Management or surgeon preference. Listed products, vendors, and catalog numbers are the reference standard; functional equivalence is acceptable.
